Additional Documentation Instructions
The following documents will be required at the time of your physical agility test. If you do not have all of these documents, we will not be able to allow you to participate in the physical agility test and your application will be considered withdrawn.
  • A copy of your birth certificate from the Bureau of Vital Statistics or citizenship papers (if applicable)1
  • A copy of your current valid driver’s license
  • DD214 Form, showing an honorable discharge (if applicable)
  • Name change documents – i.e., marriage license, court order, etc. (if applicable)
The following documents will be required after successful completion of the physical agility test and the Oral Interview Board:
  • A copy of your high school diploma, certified transcripts, or G.E.D. certificate
  • Sealed certified college transcripts (if applicable) will be required if we are unable to verify your educational achievements through a verification service.
  • A copy of your social security card2
